In regions where medicines are expensive or official distribution networks are weak, many people obtain drugs from informal sellers or unregulated markets. These channels can improve access when pharmacies and clinics are difficult to reach, but they also make it easier for counterfeit products to enter circulation. Such medicines may contain too little of the intended active ingredient, the wrong substance, or no effective medicine at all. Because packaging can closely resemble legitimate products, patients and even health workers may find it difficult to recognize that a drug is unsafe or ineffective before it is used, especially when reliable testing is unavailable.

Counterfeit medicines can cause serious health problems. Patients may fail to receive the treatment they need, allowing infections or other illnesses to worsen. The danger is especially serious when a product contains some medicine but not enough to work properly. In such cases, disease-causing organisms may be exposed to a weak dose that does not eliminate them, increasing the chance that resistant strains will develop. This can make future treatment more difficult and reduce the effectiveness of important medicines for other patients as well. The consequences can therefore extend beyond individuals who unknowingly purchase counterfeit drugs.

One response is to give legitimate medicines unique identification codes that can be checked throughout the supply chain. Manufacturers, distributors, pharmacies, and health facilities can record each stage from production to final sale, making suspicious products easier to detect. However, introducing such systems everywhere can be expensive. Small pharmacies and clinics may lack the technology, staff, or funding needed to participate fully. In addition, medicines sold through informal markets may never enter the official tracking system at all. As a result, traceability can strengthen regulated supply chains but cannot completely prevent counterfeit products from reaching consumers.
