Organ transplants can save the lives of patients whose hearts, kidneys, livers, or other organs no longer function properly. However, the number of organs available for transplantation remains far below the number of people who need them. In many countries, individuals who wish to donate their organs after death must officially register their consent in advance. This system depends on people taking action while they are healthy, yet many never complete the necessary process. Some may support organ donation in principle but postpone registration or simply be unaware of how to do it. As a result, potentially suitable organs may not become available after death.
The shortage has serious consequences for patients awaiting transplants. Because demand exceeds supply, many people remain on waiting lists for months or even years. During this time, their conditions may worsen, and some become too ill to undergo surgery successfully. Others may die before a suitable organ becomes available. Long waits can also place a heavy burden on health-care systems, since patients often require repeated treatment to remain stable. The gap between the number of willing donors and the number formally registered therefore reduces the chances that available organs can be matched with patients in time.
One proposed response is an opt-out system, under which adults are treated as potential donors unless they have recorded a decision not to participate. Supporters argue that this can increase the supply of organs by including people who favor donation but never register. However, the approach has raised ethical concerns. Some people may become donors simply because they did not understand the system or failed to record their wishes. Critics argue that consent should be explicit when decisions involve a person's body after death. If the public believes that individual choice is being weakened, trust in transplant programs could decline, potentially making families less willing to cooperate with donation procedures.
